Selecting Coverage For Your Dependents

Selecting Health, Dental, and/or Vision Coverage for Existing Dependents Such as Your Spouse or Domestic Partner

Follow the instructions below to select coverage for your Spouse or Domestic Partner only if you had enrolled them in the previous year.

Note: If you have waived any coverage for yourself, the dependent selections will automatically default to "No" and cannot be changed unless you change your own elections.

Step 1 - From the Benefits Selections screen which contains your employee elections, and click View Dependents.

Step 2 - Click the Click Here to Enroll This Dependent link in the row which contains the name of your spouse or domestic partner. 
 
Dependent Coverage

Step 3 - Populate the fields in the Edit Dependent Information section:

Enroll in Health - Only enabled if you have selected ‘Yes’ for health coverage for yourself. Click the drop-down list and select Yes or No.

  1. If you select ‘Yes’ to enroll your spouse or domestic partner in health, the Spouse/Domestic Partner Surcharge Affidavit box will appear. Proceed to Step 3.2 below. If you select ‘No’ to waive health coverage for your spouse or domestic partner, proceed to Step 4-Other Health Coverage.
  2. Spouse/DP Employed: Select Yes or No. If you select ‘Yes’, proceed to Step 3.3 below, otherwise proceed to Step 3.6.
  3. Spouse/DP County Employee: select Yes or No. If you select ‘Yes’, proceed to Step 3.5. If you select ‘No’, proceed to  Step 3.4.
  4. Spouse/DP Employer: Enter the employer’s name of your spouse or domestic partner.
  5. Employer Health Coverage Available: select Yes or No.
  6. Certification Checkbox: Check this box to certify that the spousal or domestic partner information is correct.

Step 4 - Other Health Coverage - Click the drop-down list and select Yes or No.

Step 5 - Enroll in Dental - Only enabled if you have selected ‘Yes’ for dental coverage for yourself. Click the drop-down list and select Yes or No.

Step 6 - Facility # - Only enabled if you have selected ‘Yes’ for dental coverage. Enter the same Facility # that you entered for yourself, or click the Dentist List link to view the list of dentists (dental providers), and then type the PCD # associated with the dentist in the Facility # field.

Step 7 - Enroll in Vision - Only enabled if you have selected ‘Yes’ for vision coverage for yourself. Click the drop-down list and select Yes or No.

Step 8 - Click Enroll Dependent. The dependent is now enrolled for the specified coverage.

Step 9 - Repeat Steps 2 - 8 to enroll other existing dependents and then proceed to Step 10 when you are finished. If you need to add new dependents (dependents that were not already enrolled in the previous year), then proceed to Adding Other Dependents as New Dependents.  

Step 10 - Once you have completed enrolling the existing dependents, click the Next button.

Step 11 - Click Submit Enrollment. Your 2014 Open Enrollment Confirmation Statement appears in a separate browser window.

Step 12 - Proceed to Reviewing/Printing Your Confirmation Statement to review and/or print your confirmation statement.

Selecting Health, Dental, and/or Vision Coverage for All of Your Other Existing Dependents

Follow the instructions below to select coverage for all other dependents which were enrolled in the previous year.

Note: If you have waived any coverage for yourself, the dependent selections will automatically default to "No" and cannot be changed, unless you change your own elections.

Step 1 - Navigate to the Benefits Selections screen which contains your employee elections, and click View Dependents

Step 2 - Click the Click Here to Enroll This Dependent link in the row which contains the dependent's name.
 
Dependent Coverage

Step 3 - Populate the fields in the Edit Dependent Information section:

  1. Disabled - Click the drop-down list and select Yes or No.
  2. Child age 26 but less than 30 on 01/01/2014? - If this field is enabled, click the drop-down list and select the desired option. If you select Student or Financially Dependent, an Affidavit window will appear. After you have read the affidavit, click the Acknowledge button.
  3. Enroll in Health - Only enabled if you have selected ‘Yes’ for health coverage for yourself. Click the drop-down list and select Yes or No.
  4. Other Health Coverage - Click the drop-down list and select Yes or No.
  5. Enroll in Dental - Only enabled if you have selected ‘Yes’ for dental coverage for yourself. Click the drop-down list and select Yes or No.
  6. Facility # - Only enabled if you have selected Yes for dental coverage. Enter the same Facility # that you entered for yourself, or click the Dentist List link to view the list of dentists (dental providers), and then type the PCD # associated with the dentist in the Facility # field.
  7. Enroll in Vision - Only enabled if you have selected ‘Yes’ for vision coverage for yourself. Click the drop-down list and select Yes or No.

Step 4 - Click Enroll Dependent. The dependent is now enrolled for the specified coverage.

Step 5 - Repeat Steps 2 - 4 to enroll other existing dependents, and then proceed to Step 6 when you finished enrolling the desired dependents. If you need to add new dependents (dependents that were not already enrolled in previous years), proceed to Adding Other Dependents as New Dependents.  

Step 6 - Once you have completed enrolling the existing dependents, click the Next button.

Step 7 - Click Submit Enrollment. Your 2014 Open Enrollment Confirmation Statement appears in a separate browser window.

Step 8 - Proceed to Reviewing/Printing Your Confirmation Statement to review and/or print your confirmation statement.

Adding Your Spouse or Domestic Partner as a New Dependent

If you have waived any coverage for yourself, the dependent selections will automatically default to "No" and cannot be changed, unless you change your own elections.

Step 1 - Navigate to the Benefits Selections screen which contains your employee elections. If you already have existing dependents, click View Dependents and then click Add New Dependent. If you’ve never enrolled any dependents, click Add New Dependent.

Step 2 - Populate the fields in the Personal Details section located at the bottom left corner of the screen:

  1. SSN - Type the dependent's social security number. If the dependent does not have a social security number, type 000000000.
  2. First Name, Middle Initial, and Last Name - Type the dependent's first, middle, and last name.
  3. Gender - Click the drop-down list and select Male or Female.
  4. Relationship - Click the drop-down list and select Spouse or Domestic Partner.
  5. Date of Birth - Type the dependent's date of birth (MM/DD/YYYY).

Step 3 - Populate the fields in the Enrollment Selections section:

Enroll in Health - Only enabled if you have selected ‘Yes’ for health coverage for yourself. Click the drop-down list and select Yes or No.

  1. If you select ‘Yes’ to enroll your spouse or domestic partner in health, the Spouse/Domestic Partner Surcharge Affidavit box will appear. Proceed to letter Step 3.2 below. If you select ‘No’ to waive health coverage for your spouse or domestic partner, proceed to Step 4-Other Health Coverage.
  2. Spouse/DP Employed: select Yes or No. If you select ‘Yes’, proceed to letter Step 3.3 below, otherwise proceed to letter Step 3.6.
  3. Spouse/DP County Employee: select Yes or No. If you select ‘Yes’, proceed to letter Step 3.5. If you select ‘No’, proceed to letter Step 3.4.
  4. Spouse/DP Employer: Enter the employer’s name of your spouse or domestic partner.
  5. Employer Health Coverage Available: Select Yes or No.
  6. Certification Checkbox: Check this box to certify that spousal or domestic partner information provided is correct.

Step 4 - Other Health Coverage - Click the drop-down list and select Yes or No.

Step 5 - Enroll in Dental - Only enabled if you have selected ‘Yes’ for dental coverage for yourself. Click the drop-down list and select Yes or No.

Step 6 - Facility # - Only enabled if you have selected Yes for dental coverage. Enter the same Facility # that you entered for yourself, or click the Dentist List link to view the list of dentists (dental providers), and then type the PCD # associated with the dentist in the Facility # field.

Step 7 - Enroll in Vision - Only enabled if you have selected ‘Yes’ for vision coverage for yourself. Click the drop-down list and select Yes or No.

Step 8 - Click Submit New Dependent. The dependent is now enrolled for the specified coverage.

Step 9 - Click Next.

Step 10 - Click Submit Enrollment to complete the enrollment.

Step 11 - Proceed to Reviewing/Printing Your Confirmation Statement to review and/or print your confirmation statement.

Adding Other Dependents as New Dependents

Step 1 - Navigate to the Benefits Selections screen which contains your employee elections. If you already have existing dependents, click View Dependents and then click Add New Dependent. If you’ve never enrolled any dependents, click Add New Dependent.

Step 2 - Populate the fields in the Personal Details section located at the bottom left corner of the screen:

  1. SSN - Type the dependent's social security number. If the dependent does not have a social security number, type 000000000.
  2. First Name, Middle Initial, and Last Name - Type the dependent's first, middle, and last name.
  3. Gender - Click the drop-down list and select Male or Female.
  4. Relationship - Click the drop-down list and select your relationship with the dependent.
  5. Date of Birth - Type the dependent's date of birth (MM/DD/YYYY).

Step 3 - Populate the fields in the Benefits Details section:

  1. Disabled - Click the drop-down list and select Yes or No.

  4. Facility # - Only enabled if you have selected ‘Yes’ for dental coverage. Enter the same Facility # that you entered for yourself, or click the Dentist List link to view the list of dentists (dental providers), and then type the PCD # associated with the dentist in the Facility # field.
  5. Enroll in Vision - Only enabled if you have selected ‘Yes’ for vision coverage for yourself. Click the drop-down list and select Yes or No.

Step 5 - Click Submit New Dependent.

Step 6 - Click Next if you are finished adding dependents, or click Add New Dependent to add additional dependents and then repeat Steps 2 -5 above.

Step 7 - Click Submit Enrollment to complete the enrollment.

Step 8 - Proceed to Reviewing/Printing Your Confirmation Statement to review and/or print your confirmation statement.
