Human Resources is pleased to reintroduce the Pharmacy Waiver program with a significant enhancement: Spouses/Domestic Partners will be eligible to participate. Employees and eligible spouses/domestic partners who are enrolled in the County's health insurance can benefit from the program which covers, at full cost, maintenance medications for certain disease states. The program is a huge cost savings vehicle for participants; it is strictly confidential and in compliance with HIPAA guidelines.
Employees and eligible spouses/domestic partners will need to voluntarily complete the following:
- Enroll in HumanaVitality online wellness program
- Complete the Vitality Check (Finger stick screening)
- Complete the online Health Risk Assessment
- Participate in the Disease Management Coaching (if applicable)
The program will cover the following Disease States: Asthma, Diabetes, and Cardiovascular (hypertension, chronic kidney failure, chronic obstructive pulmonary disease, coronary artery disease and congestive heart failure). The waiver will last through the end of 2015 and will cover generic and formulary drugs.
Employees and eligible spouses/domestic partners interested in participating in this voluntary waiver program will need to complete the 2015 Pharmacy Waiver program eligibility form and provide it to one of the on-site Humana Representatives for verification of the above requirements. The form will also give Humana permission to release your name and requested disease states to Employee Benefit Services so that the waiver can be submitted to your pharmacy provider, Catamaran.
Where can I complete the Vitality Check?
Employees and eligible spouses/domestic partners can complete a Vitality Check on-site during Health Fairs and other Well-Being Program events scheduled throughout the County. Employees can also complete a Vitality Check at a Concentra Urgent Care facility (appointments are recommended) or visit a Walgreens with an on-site HealthCare clinic. Please note that members will need to print out the Vitality Check form which can be found online at Humana.
