Human Resources Division is pleased to announce lower rates for dental, life insurance, and long-term disability plans effective April 1. The special open enrollment period is from March 3 - 14. An individualized enrollment form will be sent via interoffice mail to you. The enrollment form must be returned to Employee Benefit Services with any changes, additions, or cancellations by Friday, March 14, before 5:00 p.m. Please note that although coverage takes effect April 1, the lower deduction rates will reflect on 04/18/14 pay period.

What are the new changes?

- Dental PPO: HumanaDental will replace The Standard
- Dental DHMO: Will remain with CompBenefits/Humana
- Life Insurance: Minnesota Life Insurance Company will replace The Standard
- Long-Term Disability: Will remain with The Standard

What if I am already enrolled in one or all of the plans?

NO ACTION IS REQUIRED if you do not wish to make any changes to your current enrollments. There will be no gaps within your benefits or coverage(s). Your current coverage will automatically rollover and be adjusted for the new lower rates.

Purchasing Optional Life Insurance Plans
Guaranteed issue! During this special open enrollment, you are able to purchase optional term life insurance on self, spouse/domestic partner, and/or children without medical underwriting. If you are interested in increasing your optional life insurance, you will be provided a guaranteed issue of $25,000. An increase higher than $25,000 will require medical underwriting for approval. How can I find out how much optional life insurance do I currently have?
Your individualized enrollment form will be pre-populated with your current coverages. For further questions, you can contact Employee Benefit Services at 954-357-6700 or benefits@broward.org.

Pre-existing Conditions and Long-Term Disability
If you are currently not enrolled in long-term disability, you can elect to enroll without Evidence of Insurability (EOI) unless you were previously declined by The Standard as ineligible. New enrollees that have a pre-existing condition that was diagnosed six (6) months prior to the effective date of coverage will have a 12-month waiting period for coverage. The Standard representatives can provide additional information on these exclusions.

Attend a fair to learn more
Representatives will be on-site to answer questions and assist with enrollment from March 3 - 7. Staff members from Human Resources Employee Benefit Services will be at scheduled locations to assist through March 14. Attending a session will allow you to meet with your current and new insurance representatives.
