Permanent Supportive Housing

NewHart and Hart & Home are HUD funded Supportive Housing Programs designed to encourage persons experiencing chronic homelessness that have substance use or co-occurring disabilities to actively engage in the transition from homelessness.

Participants share an apartment with another program participant, each paying 30 percent of their income toward their half of the rent, with subsidy from the HUD grant if necessary. Participants are supported through case management to identify personal goals with the ultimate objective of independent housing and self-sufficiency. Each participant receives intensive case management and assistance in developing skills including:

  • Budgeting
  • Household management
  • Job attainment and maintenance
  • Substance use recovery management
  • Mental health preservation

Eligibility requirements:

  • Experiencing chronic homelessness in Broward County
  • Have a substance use disability or co-occurring disabilities  
  • All referrals must be received through Broward County’s Coordinated Assessment that can be accessed through Homeless Outreach Teams or by calling 954-563-4357
