PEER MENTOR

NATURE OF WORK

 

This is paraprofessional case work and peer support associated with a County behavioral health and/or substance abuse or addiction recovery program. Work involves the performance of supportive interaction and peer assistance under the direction of a case manager team leader and/or supervisor.   Assists with implementation of individualized rehabilitation and service plans. Supervision is provided through conferences with supervisor and adherence to established policies and procedures.

 

ILLUSTRATIVE TASKS                 Some tasks may vary slightly based on area of assignment.

 

Develops and maintains supportive, non-judgmental, trusting relationships with clients and/or consumers; may provide support to facilitate admission and discharge from health care facilities and/or addictions and/or substance abuse recovery programs.

 

Provides supportive interaction and/or intervention;  assists in carrying out individualized care, service, or treatment plan objectives; assists clients and/or consumers with activities of daily living and skill building, as needed; assists clients and/or consumers to build formal and informal support networks; assists case manager and or other staff as directed.

 

Fosters or assists clients and/or consumers achieve autonomy through individualized skill building and teaching of coping skills; help clients and/or consumers recognize and utilize their strengths to achieve community integration.

 

Provides effective rehabilitation, intervention activities, and environmental modifications.

 

Models self-advocacy and adaptive mediation techniques; encourages and facilitates community participation and integration; shares knowledge of community resources.

 

Coordinates recreational, social and leisure activities both within and outside residence; acts as initial community support until natural community supports can be developed.

 

May accompany clients and/or patients to and from health care facilities, i.e., clinics, hospitals, recreation therapy or other related activities; conducts home and field visits.

 

As directed by the team leader or supervisory staff, consults with health care personnel and other providers serving the consumer.

 

Represents the County in community meetings and advocates on behalf of consumers; attends staff and/or team meetings and case staffings as scheduled and shares known status and progress of consumers;  attends other related internal and external meetings as scheduled.

 

Provides feedback from a peer perspective regarding program service delivery.

 

Ensures and protects consumer rights and dignity.

 

Maintains confidentiality of consumer and related information as regulated under   public health laws, JCAHO standards, federal confidentiality laws (42CFR, 45CFR), and state regulations (65D-30).

 

Completes documentation in adherence to agency guidelines using basic computer-based office applications.

 

Monitors personal safety and living environment of consumers.

 

Performs related work as required.

Knowledge of various public and private community mental health and substance abuse and/or addiction recovery resources.

 

Knowledge of the personal and social effects of addiction, substance abuse, co-occurring disorders, and/or mental illness.

Knowledge of public health laws, JCAHO standards, federal confidentiality laws (42CFR, 45CFR), and state regulations (65D-30).

 

Ability to communicate effectively verbally and in writing.

 

Ability to attend to the needs of others.

 

Ability to utilize empathy skills effectively.

 

Ability to use a personal computer and basic software applications.

 

Ability to prepare correspondence, notices, schedules, and related materials as needed.

 

Ability to apply verbal de-escalation techniques.

 

Ability to serve the public and fellow employees with honesty and integrity in full accord with the letter and spirit of Broward County Ethics and Conflict of Interests policies.

 

Ability to establish and maintain effective working relationships with the general public, co-workers, elected and appointed officials and members of diverse cultural and linguistic backgrounds, regardless of race, color, religion, gender, national origin, age, marital status, political affiliation, familial status, disability, sexual orientation, pregnancy, or gender identity and expression.

REQUIRED EXPERIENCE AND TRAINING

 

Graduation from high school or equivalent certification; two (2) years of experience in peer support services or closely related activity which includes six (6) months of experience as a primary consumer of mental health or behavioral health services; or any equivalent combination of relevant training and experience.

NECESSARY SPECIAL REQUIREMENT

 

Must possess a valid Florida driver = s license. Must be able to drive and to obtain and maintain authority to drive on County business.
