Assisted Living Waiver Program provides a service continuum to functionally impaired, low income adults 60 years of age and older who are at risk of premature nursing home placement and who reside in an approved Assisted Living Facility (ALF). Consumers must meet Medicaid financial eligibility criteria and a level of care designation.

The  Assisted Living (AL) Waiver Program provides:

  • Attendant Alert Call System
  • Attendant Care
  • Behavior Management
  • Case Management
  • Chore
  • Companionship
  • Homemaker
  • Intermittent Nursing
  • Medication Administration
  • Personal Care
  • Physical Therapy
  • Occupational Therapy
  • Specialized Medical Equipment and Supplies
  • Speech Therapy
  • Therapeutic social and Recreational Service

Aged and Disabled Adult (ADA) Waiver provides a service continuum to functionally impaired, low income adults 60 years of age and older who are at risk of premature nursing home placement. Consumers must meet Medicaid financial eligibility criteria and a level of care designation. This program also offers a Consumer Directed Care (CDC) option to participants who qualify.

The ADA Waiver Program (Med waiver) provides:

  • Adult Day Health Care
  • Caregiver Support Training
  • Case Aide
  • Case Management
  • Chore, Companion
  • Counseling
  • Emergency Alert Response System Services
  • Medical Supplies
