The Human Services Department provides many services to clients and is authorized to charge fees to clients on a bundled basis (a flat fee based on any number of services provided within a specified period of time) or on an individual fee for service basis, and for services which are not otherwise paid for, in whole or in part, by Medicaid, or any other governmental agency. Bundled or individual fee for services are categorized based on long established billing guidelines by Medicaid, and other governmental entities. The following is a description of each service category and the fee associated with that service category.

Part I. Direct Services BARC |
1. Bundled Services:
A bundled service is defined as a flat fee based on any number of services provided within a specified period of time. These rates are established for Broward Addiction Recovery Division (aka BARC), Elderly and Veteran Services Division (EVSD) and Family Success Administration Division (FSAD).

4. Other Fees and Billing Practices
a. Meal Service
Broward County employees who work at or for BARC may purchase meals prepared at the Division's residential facility. The Division may make adjustments to the price of the meals as necessary, on an annual basis, to cover the cost of the meal in accordance with policies and procedures established and maintained by the Division.

The Human Services Department Director shall establish a written procedure for the purchase of such meals.
The revenues collected pursuant to this Section, shall be deposited in the General Fund Account of the BARC Division.
b. Fees for Continuing Education Units (CEU’s)
The Department or any of its Divisions may charge for classes it provides for Continuing Education Units (CEU’s) to outside participants at a fee of $16.68 per Continuing Education Unit (CEU) for internal or locally contracted trainers. If the Department or any of its Divisions' contracts with non-locally contracted trainers, a fee in the amount of $27.81 per CEU may be charged to outside participants. Outside participants are defined as anyone who is not currently employed by the Human Services Department.
Should the Department or any of its Divisions become certified as a Florida Board of Psychology or an American Psychological Association provider, CEU’s for those specialized training's may be charged at $27.81 per CEU.
If the Department or any of its Divisions receives a grant to provide training from an outside source, the cost of the CEU’s may be offset to the extent of the grant funding. The Department and its Divisions reserve the right to negotiate for trainers in exchange for class participation.
c. Billing Practices
The Department and its Divisions reserve the ability to accept cash, money order, credit cards, or bank checks as a form of payment. Personal checks will not be accepted.
d. Sliding Fee Scale and Contracted Rates
Broward Addiction Recovery Division (aka BARC) Sliding Scale
The Division has established a sliding fee scale based on the annually established U.S. Department of Health and Human Services Poverty Guidelines for the forty-eight (48) contiguous states, including adjustments for family size as published in February of each year. The client’s income level and family size determines where he or she places on the sliding fee scale.

Elderly and Veterans Services Division Sliding Fee Scale
An individual client fee will be assessed based on the current Human Services Department Fee Schedule for the delivered service not to exceed the maximum current fee. Fees are adjusted based on a sliding fee scale as specified in program guidelines promulgated by the Florida Department of Elder Affairs.
Contracted Rates
The Department/Division may apply for or receive grants or otherwise enter into agreements with outside agencies and health care providers setting forth a fixed fee for the services described above. The Department/Division Director shall have the authority to negotiate fees not to exceed 100% of the maximum published fees referred to in Section 36.1.

| A full food service inspection shall be conducted two (2) times a year at child care facilities providing full food service. Full food service shall refer to the manipulation of foods intended for human consumption by such means as washing, slicing, peeling, chipping, shucking, scooping, and/or portioning. The term also includes those activities involving temperature changes, combining ingredients, opening ready-to-eat food packages, or any other activity causing physical or chemical alterations in the food. |

| A limited food service inspection shall be conducted on an annual basis at child care facilities with food service operations, so limited by the type and quantity of foods prepared and the equipment utilized, that they pose a lesser degree of risk to the public's health, and, for the purpose of fees, requires less time to monitor.

2. Child Care and Home Facility Licensure Fee Schedule
License Fee:
- Child care facilities with up to 50 children will be charged a license fee of $81.93.
- Child care facilities with over 50 children and up to 150 children will be charged a license fee of $144.95.
- Child care facilities with over 150 children will be charged a license fee of $189.05.
- If a child care facility elects during its licensure period to change its name, change the licensed capacity of the facility, or add nighttime child care as a service, an additional fee of $63.02 will be charged.
3. Other Fees
- There shall be a $31.52 non-refundable application fee for any new prospective owner-operator of a child care facility.
- Renewal applications received less than 45 days before the expiration date of the license shall be assessed a $63.02 late processing fee.
- Family Child Care Homes will be charged $31.52.
- There shall be a screening fee for processing fingerprint results of $59.86 per person charged to child care personnel of child care facilities, including arrangements for child care.
