Community Care for the Elderly Section 

Provides a service continuum to functionally impaired adults 60 years of age and older who are at risk of premature nursing home placement.

Services may include:
  • Case Management
  • Homemaking
  • Personal Care
  • Respite Care
  • Emergency Alert Response
  • Adult Day Care
  • Monthly Caregiver Stipend
  • Consumables and Medical Supplies
Sliding scale co-payment determined by income.

Behavioral Health Section 

Provides assistance to adults 55 years of age and older who have psychiatric disabilities.

Services may include:
  • Case Management
  • In-Home/On-site Counseling
  • Outreach and Prevention Services
  • Referrals to BARC.
Sliding scale co-payment determined by income.

Veterans Services Section 

Provides free assistance to veterans, their surviving spouses, dependents  and  dependent  survivors regardlessof age or other status. Services arefocused on attainment and/or maintenance of self-sufficiency.

Services may include: 

  • Benefits and Claims Processing
  • Information, Referral and Linkage
  • Outreach

Customer Relations & Senior Services Section 

Serves as the "Welcoming Open Door" for accessing all Division services including:

Services may include:  

  • Receives all customers to simplify appropriate connections
  • Facilitates prompt screening to identify and prioritize customer needs
  • Coordinates individualized and appropriate services
  • Follows up to ensure timely and integrated quality response by Division and external agencies
  • Alzheimer's Disease Initiative:
    • Case Management
    • Respite Care
  • Consumer Directed Care:
    • Case Management
    • Monthly Care Stipend
