Memorial Hospital Healthcare’s Mammogram Van will be visiting three Broward County facilities in September on the dates below. Memorial Hospital will handle registration and participants should have their insurance information available when registering. To make an appointment, please call 954-276-5500. Participants are asked to arrive 5-10 minutes prior to the appointment.
Under the County’s health plan, an annual mammogram is not subject to a deductible or co-insurance and is provided at no cost to Humana members. Memorial Hospital will accept other insurance or self-pay for $135.
All participants will need a prescription from their physician/provider and must complete the Mammogram Questionnaire. You can contact Employee Benefits at 954-357-6700 or firstname.lastname@example.org if you have questions or require additional information.
A screening mammogram is for asymptomatic patients as a baseline or routine annual wellness exam. Please note the following restrictions: Participants must be 35 or older, have no symptoms (new lumps, masses, and/or discharge), have no personal history of breast cancer, not pregnant or nursing, and must be able to independently walk up stairs and stand alone. All participants should not wear deodorant, perfume, or body powder on the day of the exam. It is very important that participants bring prior mamograms for comparison if not done at Memorial Hospital.
