Plat Section Forms
Adjacent Right of Way Report (DOC) This form provides an outline of the elements that must appear in an Adjacent Right-of-way Report as required under the Broward County Land Development Code. This document would be used by a Title Company or an Attorney to prepare this document for submittal of a plat.
Application for Engineering Review for Chapter 177 Florida Statutes and Documentation Check List (PDF) This form provides a list of items required for submittal of an original plat mylar.
Certified Corner Record Affidavit Form(PDF) This form is used by Platting Surveyors who have used one or more existing Section corners in creation of a plat and are providing a statement that the corner and its accessories were found to be substantially as described on the most recently available Certified Corner Record on file. (To be completed when a new Certified Corner Record is not being filed.)
Certificate of Title Format (DOC)​​ This form provides an outline of the elements that are required in a Certificate of Title under the Broward County Land Development Code. This document would be used by a Title Company to prepare this document for submittal of a plat.
Consent and Dedication Form Authorizing Creation of Plat (PDF) This form is used by property owners who anticipate selling the property prior to plat recordation and can be substituted for the submittal requirement for all owners to execute the mylar.
Dade County and Palm Beach County Plat Transcript Numbers (PDF) This document lists plats that were located in Broward County but were recorded prior to October 1, 1915 (the date that Broward County came into existence). The listing includes the Dade or Palm Beach County recording information as well as the Broward County recording numbers. This document is a PDF file that can be viewed and searched using Adobe Acrobat Reader.

Plat Review Service Charge Form (XLS) This form outlines the method of calculating the FS 177 review fee.
Surveyor’s Affidavit Confirming Error on a Recorded Plat (PDF) This form is used in the event that an error or omission in the data shown on a plat is discovered after recordation. Under Florida Statutes Chapter 177.141, a professional surveyor and mapper may file an affidavit confirming error(s) and/or omission(s) on a recorded plat. In this document, the professional surveyor and mapper must describe the error or omission and the appropriate correction that should be substituted for the erroneous data.
