The Broward County Records, Taxes and Treasury Division’s Passport Application Center, in our capacity as an Acceptance Facility, receives your DS-11 Application For A U.S. Passport. Once processed, your application (along with all required documentation) is sent to the U.S. Department of State, Bureau of Consular Affairs for approval and processing. The process takes about four to six weeks if electing Routine Delivery; however, you can pay for Expedited services which will reduce the processing turn-around-time to approximately two to three weeks. Our Passport Desks open at 8:30 AM and close promptly at 2:45 PM Monday through Friday, excluding legal holidays. Our office is not able to process applications after this time. On days with a high volume of applicants, we reserve the right to close the Passport windows early, to get all applications out in that day's mail. No appointment is necessary. Please arrive to our facility well in advance, as it typically takes approximately 20 minutes per application. Please make sure all information on your application is accurate and complete, and be sure to bring all necessary items as listed below. Do not sign the application until you are directed to do so by the Acceptance Agent. If you have more than 2 applicants in your party, please be sure to arrive by no later than 2:15 PM. Applicants who arrive just prior to closing and who are not prepared will be asked to return the next business day. Our office is located in the Broward County Governmental Center in Room 114. Our address is 115 S. Andrews Ave, Fort Lauderdale, FL 33301. Metered parking is available on the North and South side of the building, and garage parking is available in the City of Fort Lauderdale parking garage located on S.E. 2nd St. Parking fees apply. Driving directions: From I-95, exit Eastbound onto Broward Blvd., then turn Right onto Brickell Ave, and then Right onto S.E. 2nd St. The entrance to the City parking garage is directly on the right.

To submit a DS-11 Application For A U.S. Passport:

Proof of U.S. citizenship:
- Certified birth certificate issued from Vital Statistics. Birth Cards or Hospital issued birth certificates are not acceptable as primary evidence. A certified birth certificate must have the registrar’s raised, embossed, impressed or multicolor seal, have the registrar’s signature, and the date the certificate was filed with the registrar’s office, which must be within 1 year of birth. All birth certificates must also include the full names of the applicant’s parent(s), the full name of the applicant, and the date of birth and place of birth. For birth certificates with a file date later than 1 year of birth, or delayed certificates, please call the U.S. Dept. of State for additional requirements at 877-487-2778.

- Previous undamaged U.S. Passport (book or card)
Please Note: Our office is not permitted to submit photocopies of your documentation, with the exception of your identification, which must be presented, also. You must surrender these original documents with your application. They will be returned to you by the U.S. Dept. of State by a traceable mailing method after processing. If you wish to keep a photocopy of your documentation for your records, please be sure to do so prior to arriving in our office.

- (Minor applicants) Parent’s ID, as listed above, is sufficient to identify a minor child. Remember, both parents must be present with the child
One passport photograph: (Our facility DOES NOT provide photo services. Please come prepared with your passport photo.)
- 2” by 2” in size
- Taken within the past 6 months, and showing current appearance
- In color
- Full face, front view with plain white or off-white background
- Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
- Taken in normal street attire
- Uniforms should not be worn, except religious attire that is worn daily
- Do not wear a hat or headgear that obscures the hair or hairline
- If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture
Glasses of any type are no longer allowed in passport photos. If you cannot remove your glasses for medical reasons, you must include a signed note from your doctor with your application
- Shirts are to be in any color other than white
Minors applying for a passport:
- Children under age 16 require both parents to be present along with the child, and provide acceptable proof of identity and evidence of relationship. If the parent’s name has changed from what appears on the child’s birth certificate, a certified copy of the document effectuating the name change must accompany the application. i.e., certified copy of new marriage license, certified copy of court order or judgment speaking to the name change.
- If only one parent is able to appear at time of application, the absent parent may execute and have notarized a DS-3053 – Statement of Consent or Special Circumstances. The completed form must be less than 3 months old, and a legible photocopy of the absent parent’s valid government issued photo ID (front and back) is required to be attached to the DS-5525.
- Children age 16 and 17 who have a valid government issued photo ID, such as a valid driver’s license (not a temporary or a learner’s permit), or valid Official Military ID may apply without a parent present if they are able to prove “implied parental consent”. This is typically proven with a parent’s check presented as payment of the U.S. Dept. of State’s fee and must have the minor child’s name and D.O.B. printed in the memo section. In some cases, the U.S. Dept. of State may require additional written proof of consent in the form of a notarized affidavit. If the minor child age 16 or 17 does not have a valid driver’s license (not a temporary or a learner’s permit) or valid Official Military ID, a parent must appear with the child and present their own valid government issued photo ID
A completed Form DS-11 Application for A U.S. Passport
- Ensure all fields are accurately completed. If a particular field does not apply, please enter N/A for Not Applicable. All applicants will be required to swear under oath of allegiance and sign the passport application in the presence of an acceptance agent under oath. Do not sign the application until you arrive in our office and are directed to do so. Blank forms are also available in our office.
- A legible photocopy of your valid government issued photo ID (front and back). Additionally, if you are submitting applications for your minor child(ren), a legible photocopy of each parent’s ID (front and back) for each application is required by the U.S. Dept. of State. If you do not come prepared with your copy(ies), our office will be happy to provide this service for you for a fee of $1 per page.
·Two Separate Payments Are Required:
- U.S. Dept. of State fee:
- The U.S. Dept. of State fee must be in the form of a check or money order only – made payable to “U.S. Dept. of State”. Neither cash nor credit card payment is accepted for the U.S. Dept. of State fee at this facility. (Helpful tip: Applications paid by money order are processed faster, as the Passport office does not have to wait for your check to clear before they begin processing.)
- Each member of your party must have a separate payment for the U.S. Dept. of State’s fees to be attached to each application.
- Broward County Board of County Commissioner’s (BOCC) fee:
- The Acceptance Facility fee – made payable to BOCC – may be paid in cash, personal check, money order or credit card (American Express, MasterCard, Visa or Discover.) Credit cards are subject to a nominal surcharge assessed to the transaction.
Applicants may choose from a variety of delivery options. The two most common delivery options selected are Routine Delivery (Option 1) and Expedited Delivery (Option 4 – expedited both ways). Routine Delivery currently has a turn-around-time of 4 to 6 weeks. Applicants who need faster delivery may elect to pay additional fees for the Expedited Delivery service. Expedited services currently have a turn-around-time of 2 to 3 weeks. Passport renewals using Form DS-82 (Application for U.S. Passport by Mail).
In order to be eligible to renew by mail using Form DS-82,
your most recent U.S Passport MUST MEET THE FOLLOWING CRITERIA:
Passport must be undamaged and able to be submitted with your application
Issued to you when you were age 16 or older
Issued to you within the last 15 years
Issued in your current name or you can legally document your name change
If your most recent passport conforms to every point of the above criteria, you are eligible to self-renew by mail directly from your home or office. Our office does not process DS-82 applications. Be sure to read and carefully follow the directions as provided in the DS-82 form. If your most recent passport is not in compliance with ALL OF THESE POINTS, you are not eligible to self-renew by mail. You must apply by using Form DS-11 and submitting at an Acceptance Facility. Applications for Name Change or Data Correction using Form DS-5504 (Name Change or Data Correction) may be obtained by visiting our office, or you may download them directly from travel.state.gov. If your circumstance meets the eligible criteria as provided on the form, you will carefully follow the instructions as provided in the form and mail directly to the U.S. Dept. of State.
You may submit Form DS-5504 IF:
- You need to change your name in your valid U.S. passport
- You need to extend the validity of a limited passport (if eligible)
- You need to correct a printing error in your valid U.S. passport
If your U.S. Passport was issued more than one year ago, you may not be eligible to use Form DS-5504.
