Who do I call to sign up for CSMS training?
The Human Services Department Help Desk is at 954/357-5956.
When does the CSMS Users Group meet?
Meetings are currently scheduled for March 19th and May 21st of 2008. Generally, meetings are held in Room 329F (conference room) within the Office of Transportation Planning. Meeting dates/times may be subjected to change. Contact the HelpDesk to confirm scheduling.
I need to report a CSMS problem. How do I describe the problem?
To expedite calls for technical assistance with CSMS, please keep the following guidelines in mind when leaving a message at the Help Desk.
Speak slowly and clearly. Leave your first and last name. (Your title is optional.) Please spell out the name.
State the name of your agency and program location – include the eight (8) digit number that appears in the lower left hand corner in CSMS. (There are two (2) sets of four (4) digit numbers.)
State the telephone number, including any extension number needed, at least two (2) times.
If you are calling about a correction, state how it appears in the system now. Then, describe what it needs to looks like. For instance, a date appears as 10/10/05 and it needs to be 10/01/05.
Describe what action you are looking for.

How many copies do I send of the Service Activity Summary Report?
A copy should be attached to the original invoice AND the copy of the invoice.
How many copies do I send of the Service Activity Detail Report (CSMS printout)?
Only one (1) copy is required. Additional copies must be shredded due to confidentiality concerns or returned to your office.
How many copies do I send of my quarterly reports?
One (1) original and two (2) copies should be sent. They must be accurate and complete. The outcome report has a line for the name and signature of the person authorizing the report. These reports need to be signed.
Where should I send my certificate of insurance renewals and audited financial statements?
As per the contract, these documents should be filed with the Human Services Department Repository, Room A-335, 115 S. Andrews Avenue, Ft. Lauderdale, FL 33301 (Governmental Center).
When preparing our agency’s audit, who should receive the request for financial information from our auditor?
These requests should be sent to the Accounting Division, Accounts Payable, Room 203, at the Governmental Center address above.
