What are Family Success Centers?

Family Success Centers are a one-stop source of social services information, referrals and services to assist individuals and families in setting goals, learning skills, and accessing the services they need to build a stronger, healthier and self-sufficient family unit.
What makes Family Success Centers so different?
The Family Success Administration Division (FSAD) of the Human Services Department and on-site provider agencies together bring services to the community in a way that is:
Family Friendly - The centers are designed to provide a respectful, inviting atmosphere where staff are friendly and truly care.
Family Centered - Staff will work with you to develop plans and find services to meet your needs.
Easy to Access - You provide your personal information just once for all services offered through the center.
Coordinated - Staff from different agencies work together on your behalf to coordinate all the social services you need.

What Documentation do I need to bring?
Those applying for emergency assistance through the Family Success Centers will need to bring the following documentation:

Photo identification for head of household |

Social Security cards for all family members |
 |
Identification for all family members (birth certificate, driver’s license) |
 |
Proof of residency (rent receipts, utility bills, school or employment records, mortgage) |
 |
Proof of income for the past 60 days or longer (pay stubs, proof of child support/alimony, award letters from SSA, Veterans, Workers or Unemployment Compensation, proof of self-employment income, tax returns) |
 |
If no income, a support statement from family or friends who have been supporting you |
 |
Proof of assets, if any (bank/credit union statements, CDS, retirement funds, cash value of life insurance, stocks, bonds, trust funds, ownership of property) |
 |
Proof of disability (doctor’s statement, disability determination or payments) |
 |
Status of any other assistance applied for (WAGES, SSI/SSA, Unemployment or Workers Compensation |

Proof of expenses for the past 30 days (receipts) |
 |
Proof of unpaid bills (rent or mortgage, utility, eviction or foreclosure notices, collection notices) |

Status of any pending legal action |
