CASE MANAGEMENT AIDE
NATURE
OF WORK
This is para‑professional
case management work assisting Case Managers and/or clinical staff in a County social
services facility or office. Performs a variety of routine tasks assisting with
program participants needs, based on area of assignment. Prepares and maintains
case activity reports. Work may also include conducting site monitoring visits,
maintaining and updating contract files, participating in meetings, coordinating
projects, and/or transporting program participants to service locations. Supervision
is received from a professional supervisor who reviews work for conformity with
established policies and procedures.
ILLUSTRATIVE TASKS Tasks may vary based on area of assignment. Tasks are presented in no
particular order. Nomenclature for program participant varies among the
agencies using this job classification, e.g., client, consumer, patient. For
the purpose of this classification specification, program participant will be used.
Assists case
workers or other professional or clinical staff with program
participant-related activities.
Assists in
preparing case activity notes, monthly reports, or other case management
documentation.
Maintains
case files; may maintain and update contract files and/or assist in data collection
and completion of surveys, fact sheets and questionnaires; may participate in
case staffings.
Maintains
confidentiality of program participants and related information as regulated
under public health laws, JCAHO standards, federal confidentiality laws (42
CFR, 45CFR) and state regulations as required for the area of assignment.
May conduct
home and field visits as part of case plans; may transport program participants
to and from service appointments; may accompany program participants to various
appointments, including self-help meetings, as directed by a supervisor.
May liaise
with elders, care-givers, and others to monitor and ensure appropriate care
planning.
May track
allocation and spending of program-specific funds and collect data for quality
assurance programs and outcome measures.
May prepare
evidence film for processing; may distribute food and bus passes.
May receive
program participants; may provide program or other information to the public
and program participants, in person or by phone; may provide service or program
access or in-take information to or obtain information from program
participants, in person or by phone.
Provides
clerical and/or administrative support and para-professional support for
professional case management staff, including filing, answering telephones,
logging-in program participants, processing eligibility and other paper work,
preparing documents or completing forms and/or other clerical, administrative
and/or paraprofessional duties as required by the area of assignment.
Uses computer
based office applications.
Performs related
work as required.
KNOWLEDGE,
ABILITIES AND SKILLS May vary slightly based on area of assignment. Presented in no particular

Knowledge of
basic social case work procedures, practices and methods.
Knowledge of
social services offered by the County and other local community services.
Knowledge of
addiction and recovery and community support groups as required by the area of
assignment.
Some
knowledge of public health laws, JCAHO standards, federal confidentiality laws
(42CFR, 45CFR, and state regulations (65D-30) as required for the area of
assignment.
Ability to
communicate effectively with the public, co-workers, and program participants
of all ages in person and by telephone.
Ability to
react quickly and calmly in emergency or crisis situations.
Ability to
establish rapport with program participants under adverse conditions.
Ability to
use a personal computer.
Ability to
prepare reports, case notes, correspondence, and other documents.
Ability to
serve the public and fellow employees with honesty and integrity in full accord
with the letter and spirit of Broward County's Ethics and Conflict of Interest
policies.
Ability to
establish and maintain effective working relationships with the general public,
co-workers, elected and appointed officials and members of diverse cultural and

REQUIRED
EXPERIENCE AND TRAINING
Graduation
from high school or equivalent certification; two (2) years of experience
working in a social service agency, social service or community program, or
community support group related to the area of assignment; including six (6)
months of experience in clerical work; or any equivalent combination of
relevant training and experience.
NECESSARY
SPECIAL REQUIREMENT
Depending
upon area of assignment, may require possession of a valid Florida Driver's
License and ability to drive and to obtain and maintain authorization to drive
on County business.
