Affidavit of Citizenship

This document is used for reentry to the United States when the person does not have a Voter's Registration Card or if no birth certificate is available. It is most commonly used by people who are going on a cruise. It must be completed in this office and the customer must provide picture identification, such as Driver's License or picture id.card. The fee for this document is $5.00.
