Instructions for Completing the Tourist Development Tax Registration Form

1. RENTAL PROPERTY INFORMATION: Enter the actual physical location (street) address of the rental property. A post office box is not an acceptable address.
   
2. OWNER INFORMATION: Enter the owner's name (i.e., individual, partner, corporate) address and telephone number.
   
3. DEALER INFORMATION: Enter the dealer's name (i.e., operator, property manager), address and telephone number. The dealer's information relates to those individuals responsible for the business aspects of the rental property. If the owner acts as the dealer, enter "Same."
   
4. BANK INFORMATION: Enter the name of the bank, account number, address and telephone number where the business bank account is located.
   
5. ENTER your Federal Tax ID number or social security number. ENTER your State of Florida Sales Tax number. ENTER your State of Florida Filing Frequency, i.e., Monthly, Quarterly, Semi-Annual, Seasonal or Annual.
   
6. TYPE OF BUSINESS: Place a check mark on the appropriate line. If "Other", please state the type in the space provided.
   
7. TYPE OF RENTAL: Place a check mark on the appropriate line. If "Other", please state the type in the space provided.
   
8. NUMBER OF RENTAL UNITS: Indicate the number of rental units located at the rental property address.
   
9. RENTAL MONTHS: Indicate the months during which this property is rented.
   
10. BROWARD COUNTY FILING FREQUENCY: Indicate the frequency in which you wish to make your tax payments, i.e., Monthly, Quarterly, Semi-Annual or Annual.
    
11. SELECT MAILING ADDRESS: Place a check mark on the line indicating the address you wish the tax returns and related information to be mailed each year. 1 is the Rental address, 2 is the Owner address and 3 is the Dealer address.
    
12. COUNTY OCCUPATIONAL LICENSE NO: Enter your Broward County Occupational License Number.
    
13. APPLICANT'S NAME: All forms must contain the printed name and signature of the apporpriate authorized person.
