Alzheimer’s Disease Initiative provides a service continuum to functionally impaired residents, 60 years of age and older who have received a diagnosis of Alzheimer’s or dementia related diseases who may be at risk of premature nursing home placement. Sliding scale co-payment determined by income.

Services may include:

• Case Management
• In Home Respite Care
• Care Giver Support and Training
• Consumables and Medical Supplies

Eligibility qualification criteria for the services provided by this program are determined by factors such as age, diagnosis and availability of funding.​​​
