Home Care provides a service continuum to functionally impaired, seniors 60 years of age and older, who are at risk of premature nursing home placement and reside with a full time caregiver.
Services may include:
- Case Aide
- Case Management
- Basic Subsidy Stipend to Care Givers
- Subsidies for Specialized Services to Assist Caregivers.
