​​This webpage contains information on the Water and Wastewater Services (WWS) Utility Connection Permit (UCP) forms required to obtain WWS release for service. The UCP is the process by which WWS allows connection to its potable water, wastewater and/or reclaimed water systems. Click here for more information on the UCP.

In addition to the forms listed below, a Release of Lien is required from all contractors and suppliers of material and/or labor used directly or indirectly in the prosecution of the work required by the UCP.  These releases are to be on forms supplied by the UCP Permittee or contractor or supplier.

An Affidavit form that all persons, firms or corporations who furnished labor or materials used directly or indirectly in the prosecution of the work to be performed by the UCP has been paid.

A Bill of Sale Absolute form for all Permittee right, title and interest to all of the water and wastewater facilities constructed in the rights-of-way and/or easement, and any other portions to be turned over to the County for maintenance.

A Final Release of Liens form releasing all liens which the Permittee might have on the water/ wastewater facilities.

An Indemnification Agreement form which is required only for projects with fences, decorative bricks or other obstructions in rights-of-way or easements occupied by WWS facilities.

A one year Surety Bond (warranty) form covering all items built or altered which is to become Broward County property.

A Utility Maintenance License form must be used in lieu of an easement when the Permittee is not a governmental agency reporting to the Broward County Administrator and the project was constructed on Broward County Government owned property.

The following documents are not required when the Permittee is a governmental agency reporting to the Broward County Administrator:
